You have 40 employees due for mid-year reviews in the next three weeks, the data lives across spreadsheets, calendar reminders, and a shared drive folder nobody updates. This template consolidates every employee, their active goals, their review status, and their manager’s rating into a single linked Airtable base — and surfaces a ready-to-send completion report for your CHRO every Friday. It is not for HR teams building a full HRIS replacement; it is for the HR Manager who needs accurate review data by end of week.
What this template does
It replaces the manual process of cross-referencing an employee roster spreadsheet against a separate review-status tracker and then writing a summary email by hand. When you open it on day one you see three linked tables: Employees, Review Cycles, and Goals. After one week of use the base surfaces a filtered view of every review that is overdue, a per-manager completion rate, and a rollup of average performance ratings across departments — covering up to 150 active employees across a single review cycle.
Template structure
Table 1: Employees
- Employee ID (Autonumber): Auto-generated (e.g.
EMP-0041). Never edit this field. It is the unique key that links every goal and review record back to this person. If it is blank, the linked records in the other two tables will orphan. - Full Name (Single line text): Example:
Priya Nair. Enter exactly as it appears in your payroll system. Mismatches here break the mail-merge export used for review invitation emails. - Department (Single select):
Engineering/Sales/Marketing/Operations/Finance/HR/Customer Success. Set this on the day the employee record is created. The Department Completion Rate grouped view depends entirely on this field being populated — a blank department drops that employee from the summary report. - Manager (Linked record → Employees table): Link to the employee’s direct manager record. Example: link
Priya NairtoDaniel Osei. This powers the Manager View, which shows each manager only their direct reports. If left blank, the employee appears in no manager’s filtered view and their review will not be assigned. - Employment Type (Single select):
Full-time/Part-time/Contractor/Intern. Contractors and Interns are excluded from the Q2 Rollup view by a filter on this field — do not leave it blank or they will inflate your permanent headcount rating averages. - Start Date (Date): Example:
2024-03-15. Used by the Tenure formula field. Employees with under 90 days tenure are automatically flagged in the Probation flag field — if Start Date is missing that flag never fires. - Tenure (Formula — computed):
DATETIME_DIFF(TODAY(), {Start Date}, 'days'). Read-only. Drives the Probation Flag field. Do not edit. - Probation Flag (Formula — computed): Returns
Under 90 daysif Tenure is below 90, otherwise blank. Read-only. Used to filter probationary employees out of the standard mid-year review cycle. - Active (Checkbox): Checked by default. Uncheck when an employee leaves. The main Employees view filters to checked records only — unchecking immediately removes them from all active-cycle views without deleting their historical data.
Table 2: Review Cycles
- Cycle Name (Single line text): Example:
Mid-Year 2026 — Engineering. One record per employee per cycle. Name it consistently; the CHRO summary report groups by this field. - Employee (Linked record → Employees): Link to the exact employee this review covers. Example: link to
Priya Nair. If blank, this review record floats unassigned and will not appear in any manager’s view. - Cycle Type (Single select):
Probation (90-day)/Mid-Year/Annual/PIP/Promotion Review. Set this when you create the record. The Overdue by Cycle Type view groups on this field — a wrong value here surfaces a PIP review inside the standard mid-year completion report, which is a compliance risk. - Review Due Date (Date): Example:
2026-07-25. Set this the day you open the cycle. The Overdue Reviews filtered view shows every record where Review Due Date is before today and Status is notComplete. This is the view you screenshot for the Friday CHRO update. - Status (Single select):
Not started/Self-assessment sent/Manager draft in progress/Calibration pending/Complete/On hold. Change toSelf-assessment sentthe moment you trigger the employee email — not when you intend to. Change toCompleteonly when the signed review PDF has been uploaded to the Review PDF attachment field.On holdis for employees on leave; it removes the record from the Overdue view. - Self-Assessment Submitted (Checkbox): Check this when the employee’s self-assessment lands in your inbox or HRIS. The Self-Assessment Overdue view filters on this field alongside Review Due Date — it shows every employee whose self-assessment is not in and whose due date is within 7 days.
- Manager Rating (Single select):
1 — Below expectations/2 — Partially meets expectations/3 — Meets expectations/4 — Exceeds expectations/5 — Outstanding. The manager enters this during calibration. If blank when Status isComplete, the Department Average rollup in the Employees table will undercount — chase the manager before closing the record. - Overall Score (Number): The numeric version of Manager Rating, entered as
1through5. Example:4. This is the field the Department Average Rating rollup reads from. Yes, it duplicates the select — the select is for human readability, the number is for the rollup formula. - Calibration Notes (Long text): Free-text field for notes from the calibration session. Example:
Discussed with Sales leadership 2026-07-10 — rating confirmed at 4. Not required, but if a rating is ever challenged by an employee this field is your audit trail. - Review PDF (Attachment): Upload the signed PDF here before changing Status to
Complete. If this is empty and Status isComplete, the record will pass the completion filter but you will have no document — a serious compliance gap. - Reviewer (Linked record → Employees): Link to the manager conducting the review. Example:
Daniel Osei. Drives the By Reviewer workload view, which shows how many open reviews each manager is carrying. If blank, the workload view cannot distribute correctly.
Table 3: Goals
- Goal Title (Single line text): Example:
Increase CSAT score from 78% to 85% by Q3 2026. One record per goal per employee. Be specific — vague goals cannot be rated objectively at review time. - Employee (Linked record → Employees): Link to the employee who owns this goal. If blank, the goal is invisible in the Employee Goals view and will not be pulled into that employee’s review record.
- Review Cycle (Linked record → Review Cycles): Link this goal to the cycle where it will be evaluated. Example: link to
Mid-Year 2026 — Engineering / Priya Nair. This allows the review record to display a count of linked goals via the Goals Count rollup. - Goal Category (Single select):
Performance/Development/Behavioural/Project/Compliance. Used to group goals in the Goal Type Breakdown view. If blank, the goal falls into an uncategorised bucket that the CHRO summary ignores. - Target Date (Date): Example:
